CPU Record Application

To claim a CPU national record, you must:
      a) Complete this form within 7 days of the contest, or the last day of a multi-day event.
      b) Send in the Application Fee of $75.00 within the same period (ONE fee only, regardless of the number of records set)

Fee's are paid directly to the CPU, online via Paypal, or by mail (postmarked within 7 days). The page returned after submitting this form explains both options, or click this link to go there directly.
