Any lifters that wishes to claim a CPU national record, must:
a) Complete this form within 60 days of the contest (as soon as possible after the meet is advisable)
b) Send in the Application Fee of $75.00 within 60 days of the contest date.

Fee's must be paid to your provincial association, not directly to the CPU. Contact them as soon as possible, see this page for contact info. (Some provinces may not collect the fee's from the lifters, they will advise).

Note: Ontario Lifters should use the OPA record application form on the OPA site, instead of this form. The OPA form will serve as application for both National and Ontario Provincial records)
